File a Consumer Complaint  

To file a consumer complaint:

  1. Navigate to the File a Consumer Complaint page by clicking the How to File a Complaint link on the left menu.

  2. The File a Consumer Complaint page will display. This page provides a map image and links to each state insurance department. From this page, you can file a complaint directly with your state insurance department.   

  3. Click on the state or the corresponding text link for the state where the insurance policy that the complaint refers to was written. This may be different from the company's home state or the state where you live. 
     
  4. If the state where you are submitting the complaint has an online complaints process, you will be taken to that state's complaints web site. Follow the instructions provided to submit the complaint.    

  5. If the state where you are submitting the complaint does not have an online complaints process, the File a Complaint form will display. Answer the questions on the form as thoroughly as possible.    
     
  6. After you have completed all the items on the complaint form, click the File Complaint button at the bottom of the form. The complaint will be submitted directly to the person or department responsible for handling complaints in the selected state.
